TANDEM365 is a complex medical case management and community paramedicine program delivering innovative, participant-centered care. We support individuals with complex medical, behavioral health, and social needs through in-home and community-based services designed to:

  • Help participants remain safely in their homes
  • Reduce unnecessary emergency department visits and hospitalizations
  • Improve navigation of the healthcare system and community resources

What You’ll Do as an RN Navigator

As an RN Navigator, you will provide community-based clinical case management to participants with complex medical and psychosocial needs. You will conduct in-home and telephonic assessments, develop individualized plans of care, and partner closely with the interdisciplinary team to support participants across the continuum of care.

Key Responsibilities:

  • Conduct in-home and phone-based clinical assessments to inform individualized plans of care
  • Develop and monitor person-centered care plans in collaboration with participants and the care team
  • Coordinate care across primary care, specialty providers, hospitals, hospice, and community agencies
  • Assess health literacy and provide participant-centered education
  • Empower participants through self-management education and goal setting
  • Facilitate challenging, goal-focused conversations with participants and families
  • Support transitions of care, including hospice when appropriate
  • Partner with the care team to prevent unnecessary hospital utilization
  • Maintain timely, accurate documentation in electronic systems
  • Participate in on-call coverage as part of a shared team rotation

Education & Experience Requirements

  • Active Registered Nurse (RN) license in the State of Michigan
  • Minimum of 2 years of nursing experience
  • Experience in community-based care, case management, home care, population health, or care coordination strongly preferred
  • Experience supporting individuals with chronic medical conditions and behavioral health needs preferred
  • Knowledge of community resources and care coordination across settings
