Case Manager

Description:

The Council on Substance Abuse (COSA) is seeking experienced Case Managers for its Hope 4 Heroes and Families Program team. The program is a 20-county-wide initiative aimed at addressing the overall health and wellness needs of Veterans and their families. The Case Manager should have demonstrated experience working with Veterans, completing effective case management services, conducting outreach and training activities, and coordinating community events. Extensive travel is required in the assigned counties. The Case Manager reports to a Program Manager.

Requirements:

CORE JOB REQUIREMENTS

  • Provide case management services to veterans and family members
  • Establish and maintain working relationships with other agencies/organizations. Assist and refer individuals in need of substance use, mental health, and other services
  • Coordinate and facilitate meetings, workshops, seminars, and other activities to provide training and education.
  • Coordinate and implement community events
  • Report progress and activities to the program director and program coordinator regularly.
  • Conduct himself/herself in a respectable manner, whether on or off the job.
  • Plan, organize, and handle multiple tasks simultaneously and meet deadlines in a fast-paced environment; excellent attention to detail and follow-through.

ESSENTIAL JOB QUALIFICATIONS

  • Must have a minimum of one year of experience providing at least one of the following: substance abuse prevention or treatment services, mental health, suicide education, health promotion services, school services, human services or extensive working experience in the fields of substance abuse prevention or treatment for more than three years. This experience may be cumulative.
  • Demonstrated experience in community outreach and knowledge of community resources
  • Demonstrated experience in group facilitation and public speaking.
  • Advanced user of Microsoft Office, basic knowledge of other software applications (e.g., database, desktop publishing, etc.).
  • Preferred knowledge of substance abuse, mental health, suicide, risk and protective factor model; the principles of human behavior modification; substance abuse prevention and treatment terminology, methodology, organization, and practices.

PHYSICAL DEMANDS/WORKING CONDITIONS

  • Occasionally sits at a desk and lifts or moves objects weighing up to 30 pounds
  • Works for sustained periods, maintaining focused attention to detail.
  • Accommodation may be provided for some of these physical demands for otherwise qualified individuals who require and request them.
  • Flexibility and ability to work evenings and weekends if the Executive Director
  • Work duties require frequent traveling using agency and/or personal vehicle; mileage reimbursement is provided.
  • Valid driver’s license and proof of automobile insurance required.
