Position Overview

The Case Manager plays a vital role in ensuring residents receive the services, resources, and support they need to succeed. This position is responsible for coordinating care, maintaining compliance with licensing standards, and helping residents transition successfully during their time in care.

Key Responsibilities

  • Coordinate and transport residents to appointments (medical, dental, educational, psychiatric, court, and family visits).
  • Complete admission assessments, service plans, and discharge/transfer summaries.
  • Monitor, review, and approve resident restrictions as required.
  • Maintain accurate resident records and documentation.
  • Administer and record medications.
  • Support campus census goals by filling vacancies within 48 hours.
  • Oversee psychiatric and medical hospitalizations, including all steps of the process.
  • Collaborate with social service agencies and outside providers.
  • Participate in staff training and performance review processes.
  • Ensure daily program safety, cleanliness, and compliance with licensing standards.
  • Perform other duties as assigned.

Qualifications

  • Bachelor’s degree in Social Work, Psychology, or related field.
  • Minimum 2 years of experience in residential treatment or social services.
  • Knowledge of state licensing standards
  • Strong organizational, documentation, and communication skills.
  • Ability to remain professional in high-stress and emergency situations.
  • Must be able to work at least 50 hours per week and be on call 7 days a week.

Job Type: Full-time

Pay: $41,000.00 - $45,000.00 per year
