Case Manager – New York Team

Golden Care Therapy

Golden Care Therapy is seeking a driven, compassionate, and highly organized Case Manager to join our growing New York team. This role is ideal for someone who is passionate about supporting families and staff, thrives in a fast-paced environment, and excels at communication, problem solving, and relationship-building.

As a Case Manager, you will serve as a critical connection between our families, clinical teams, and administrative staff—ensuring high-quality service delivery, clear communication, and smooth day-to-day operations.

Key Responsibilities

  • Serve as the primary liaison between the office, families, and clinical staff

  • Coordinate, monitor, and track client reassessments and authorizations

  • Review and monitor weekly billing trends to ensure accuracy and follow-up

  • Conduct monthly care calls with families to provide support, address concerns, and maintain strong relationships

  • Assist with schedule changes, staffing needs, and case-related concerns

  • Proactively identify issues and collaborate with internal teams to develop effective solutions

  • Track, organize, and maintain all required client documentation in compliance with company and regulatory standards

Skills & Qualifications

To succeed in this role, you should bring both heart and hustle. The ideal candidate will have:

  • Strong administrative experience with proficiency in Excel and Outlook

  • Excellent phone etiquette, call management skills, and confidence speaking with families and staff

  • Outstanding problem-solving abilities with a proactive, solution-oriented mindset

  • Exceptional time management and organizational skills

  • Knowledge of Applied Behavior Analysis (ABA) and an understanding of behavioral health concepts and interventions

  • Strong communication and interpersonal skills, particularly when working with individuals with Autism Spectrum Disorder (ASD) and their families

  • Ability to remain calm, professional, and empathetic in high-pressure or sensitive situations

  • Bilingual in Spanish is a plus
