Job Summary
We are seeking a dedicated and compassionate Case Manager to join our dynamic team. In this vital role, you will coordinate and deliver comprehensive support services to individuals and families facing complex challenges. Duties

  • Conduct thorough assessments to identify clients’ needs across behavioral health, social work, child welfare, and workforce development areas
  • Develop personalized care plans that incorporate crisis intervention, addiction counseling, and motivational interviewing techniques
  • Provide ongoing support through coaching, mentoring, and crisis management to help clients navigate challenges effectively
  • Connect clients with community resources such as Section 8 housing, Medicare services, or childcare programs to promote stability and independence
  • Conduct community outreach and engagement activities to connect with individuals in need of services, build relationships with community partners, and increase awareness of available programs
  • Support outreach initiatives by assisting with community events, referral coordination, and collaboration with local agencies to expand access to services
  • Monitor progress regularly, adjusting care strategies as needed to ensure positive outcomes
  • Document all interactions meticulously in compliance with organizational policies and regulatory standards
  • Collaborate with multidisciplinary teams including healthcare providers, social workers, and community agencies to coordinate comprehensive care

Qualifications

  • Proven experience in social work, behavioral health, or related fields with supervising or mentoring responsibilities preferred
  • Strong skills in crisis intervention, crisis management, and addiction counseling
  • Knowledge of child welfare systems, Section 8 housing programs, Medicare, and workforce development initiatives
  • Excellent communication skills with the ability to motivate and empower clients through motivational interviewing techniques
  • Demonstrated ability to handle sensitive situations with professionalism and empathy
  • Experience working with diverse populations including children and families involved in childcare or child welfare programs
  • Relevant certifications or training in behavioral health or social services are highly desirable

Pay: $26.11 - $28.45 per hour
