Case Manager- Outreach to Homeless Program (PATH) - $19.28/Hr
Closing Date: 03/23/2026
FT 40 Hr/wk Provides outreach and engagement services to meet basic shelter, food, and clothing needs for Outreach to Homeless Program participants. Responsibilities include street outreach throughout Eastern Connecticut, canvassing targeted areas, assisting with finding housing, linking participants to resources such as social, educational, and vocational services, as well as fuel assistance, social security, and clinical services. Promotes activities in skill development such as making and keeping important appointments with healthcare providers, budgeting, and housing. Documentation of services, and charts in the Electronic Medical Records System (EMRS).
