NineLine Veteran Services CASE MANAGER SSG Fox Grant
Full Time Travel Position – 9 county region Clallam, Island, King, Kitsap, Mason, Pierce, Skagit, Snohomish and Thurston
$43,000 - $53,000 depending on experience
Under the Staff Sergeant Parker Gordon Fox Suicide Prevention Grant Funding, the Case Manager is responsible for the provision and coordination of suicide prevention services for eligible Veterans at risk of suicide and their families. The Case Manager provides services through ongoing support; expertise through assessment; planning; program implementation and evaluation of individual client needs.
ESSENTIAL JOB FUNCTIONS: Provide direct and comprehensive suicide prevention case management to Veteran households as needed
· Assessment of suicide prevention services needed
· Develop and monitor service plans
· Establish linkages with appropriate agencies and service providers in the area to help participants obtain needed suicide prevention services
· Provide referrals to participants and related activities
· Maintain accurate records of all case management activities
· Inform supervision of crises and emergencies as they arise
· Complete all case notes in a professional, timely manner
· Provide case management to Veterans and family members from various socio-economic, cultural, ethnic, educational, and other diversified backgrounds
· Assist in designation of resources and services according to client need
· Partner and collaborate with Peer Support Specialists, Nineline staff, Veteran program staff (VA, SSVF, GPD, HVRP) and other departments as appropriate
· Establish and maintain relationships with external resources and referral organizations
· Provide consultation/education to Veterans and families on community resources and VA benefits
· Maintain awareness of suicide prevention treatment options
· Participate in required program meetings/conferences/trainings
· Develop and maintain professional standards of service, coordination of suicide prevention services with other programs, representing the program in contacts with the community and having substantial accountability for program effectiveness and evaluation
· Use clear and accurate communication skills (both verbal and written) to facilitate communication with the client
· Conduct evaluations at time of intake and program exit utilizing designated assessment tools, including screenings and questionnaires
· Establish and maintain community resources
· Perform other duties and responsibilities that may be agreed upon with the supervisor as needed for the benefit of the Veteran, family, and quality of services provided
REQUIRED KNOWLEDGE, SKILLS AND ABILITIES

· Knowledgeable resource navigator: organized and aware of available resources
· Effective communicator: able to analyze situations/inform rapidly; clear and thorough written and verbal communication with participants, their families and internal/external staff

· Self awareness and appropriate boundary setting: know when to consult/debrief, follow policies, and maintain self care needs
· Familiarity and willingness to learn about mental illness, trauma, substance abuse, physical disability and other common barriers for the served population, and an ability to work with these individuals
· Knowledge of the fundamentals of grants and contracts
· Ability to work flexible hours and travel as required
· Knowledge of advocacy, community service and referral
· Excellent organizational skills and attention to detail; Strong analytical and problem-solving skills
· Ability to organize and perform work independent of direct supervision
· Ability to use sound judgment in crisis situations
· Ability to work effectively with individuals of varying incomes, skill levels, experiences, and cultures
· Ability to manage multiple tasks simultaneously through effective planning
REQUIRED EDUCATION AND EXPERIENCE:
· Bachelor’s in Counseling or Social Work with experiencing in mental health, substance abuse, Veterans services, and/or case management.
· Availability to work a non-traditional schedule that may include additional hours, in a limited capacity
· Travel will be required
· Must have a valid driver’s license and acceptable driving record
· Must have quality interpersonal, written, and verbal communication skills
· Experience with Microsoft
· Strong organizational skills and ability to multi-task
· Ability to speak and write clearly

Physical Requirements:

The vision requirement includes close and distant vision. Occasionally required to lift up to 10 pounds. Environmental Working Requirements:
Work is done in either an on-site or remote environment. Assignments may require work to be done on an individual basis or in teams with members at various staffing levels. The work environment, at times, may subject staff to behavioral and medical events that could be physically hazardous.
The ability to work various shifts and irregular hours to meet agency’s needs; Attendance on scheduled shifts and participation in mandatory meetings and trainings. Must maintain a valid driver’s license and driving record that satisfies insurability requirements
