Job Summary: Case Manager helps TWR veterans to recenter, refocus, rebuild, and change their lives by assisting them in their transition out of homelessness, substance abuse and any other barriers to success they may be battling. We empower residents by recognizing they are capable, have strengths and have resources that can help them take control of their lives. We treat residents respectfully, assisting them in identifying their needs and building on their strengths, while supporting them in meeting their goals. Benefit delivery is integrated with self-sufficiency-enhancing services. The desired outcome of case management is increased stability and self-sufficiency through individualized case management. Case management takes a team effort. The team consists of the resident, the case manager, other TWR staff, and community partners who have a mission to serve and come along side our veterans to change their life

Primary Duties & Responsibilities:

● Assessment of residents to identify situation, needs, motivations, strengths, and barriers

● Conduct ongoing resident assessment, beginning with their first contact with TWR and continuing through changes in programs and client circumstances

● Collaborate with other staff members in assessing further resident needs as needed

● Assist resident with development of a service plan, which will be individualized to identify services that support each resident's self-sufficiency goals and are based on the resident's strengths, needs and abilities

● Provide residents with information about available programs, resources, and program requirements to support them in making informed choices about recovery and lifestyle

● Address care plan issues with client, TWR staff members, and other partners to ensure the highest quality of lifestyle recovery

● Coordinate care with community partners if those partners also have a care or service plan with the client. (For example: child welfare, vocational rehabilitation, community action agencies, housing, etc.).

● Provide continual updates and evaluations of care plans, based on client needs and information gained through ongoing screenings and assessments

● Make frequent one-to-one contact with residents (at least weekly) based on the needs of the resident and care plan

● Ensure that all care plans reflect current resident abilities and the services needed to meet short and long-term goals

● Document care plans, progress, strategies, and resources in the correct format, using case management software and forms; maintain current care plan information in resident files

● Instill a desire of working towards self-sufficiency in all TWR residents

● Promote the concept of accountability and resident ownership of care plans, encouraging residents to assist staff in the development of the plan

● Inform residents that care plans are developed based upon resident goals, strengths, barriers, and resources

● Instruct TWR residents that they are accountable for their progress toward goals

● Enable residents to become proactive so they learn to use resources to meet their own needs

● Utilize a re-engagement process to reconnect the resident with their care plan goals and steps needed to address self-sufficiency, to establish whether the resident can complete their care plan, and to address any barriers to participation

● Hold residents accountable for actions and take appropriate steps for reprimand and/or program termination if it has been established that the resident is willfully noncompliant

● Provide Care Coordination using the brokering concept, which means resident needs are identified, assistance is provided in accessing services to address their needs, and follow-up is provided, based on results of the referral and interactions with the service provider

● Collaborate with residents, other staff, partners, and the community to provide services that ensure our resident's success

● Teach residents how to become their own broker so they may access services independently when needs arise

● Treat all TWR residents with the respect and dignity that honors their differences

Positional Requirements & Qualifications:

● Education & Experience:

o HS Education or GED equivalent

o Associates Degree in social work or related field is preferred but not required

o Valid Texas Driver License and clean driving record

● Skills & Abilities:

○ Ability to self-manage in an efficient and productive manner

○ Effective time-management

○ Communication and customer service skills

○ Self-Motivated

● Clear background check and drug/alcohol screening

Essential Physical Functions:

● Some driving

Job Type: Full-time

Pay: From $42,000.00 per year
