Pay: $16.50 - $18.00 per hour

Case Manager QUALIFICATIONS

  • Must be at least 21 years old.
  • Must be able to pass a thorough background check, and obtain a Level 1 Fingerprint Clearance Card.
  • Education: High School diploma or GED required, plus a minimum of two (2) years of combined education and/or relevant experience in human services, behavioral health, social services, or a related field.
  • Experience: Experience with youth and/or behavioral health preferred.

Case Manager Overview:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Reasonable Accommodations may be made to enable qualified individuals with disabilities to perform the essential functions.

  • Follow up with Admissions Coordinator to ensure consent forms are signed
  • Orientate the students to MMA expectations, intake packet and campus
  • Update student info in database on intake and through out student’s stay at MMA
  • Provide transportation to current students to court, home passes, etc, as needed
  • Appear in court as needed, telephonic or in person
  • Relay information to caseworkers, good communication between MMA and agency
  • Participate in CFT meetings and act as a liaison for MMA and outside agency
  • Assist Marketing/Admissions Department when needed
  • Responsible for Clinical charts (correct order, needed paperwork, review monthly)
  • Facilitate students staffings (service plans, signature pages, notes)
  • Compile and submit monthly referring/placing agency reports
  • Send information as requested to agencies, etc.
  • Assist student’s in preparing for discharge in conjunction with agency guidelines
  • Any other duties assigned by the supervisor
