CASE MANAGER

Responsibilities:
Licensed Clinical Case Manager (CCM)- OTP Clinic
The Licensed Clinical Case Manager (LCCM) provides comprehensive clinical case management services focused on diversion and re-entry support for incarcerated individuals with co-occurring mental health and substance use disorders. The role involves assessment, treatment planning, coordination of care, and advocacy to facilitate successful community reintegration while reducing recidivism. The position requires collaboration with correctional facilities, community agencies, and healthcare providers. The Licensed Clinical Case Manager (LCCM) will provide project oversight, clinical assessments, and brief interventions to the participants in the program. The LCCM will also be responsible for holistic comprehensive case planning, record keeping, and providing necessary information to the researcher. The LCCM will utilize evidence-based and best-practice interventions to include Cognitive Behavioral Therapy, Motivational Interviewing, and Trauma-informed care, tailored to the unique needs of our diverse population.

Responsibilities:
  • Conduct comprehensive biopsychosocial assessments of incarcerated individuals to identify mental health, substance abuse, and psychosocial needs by conducting client screenings and interviews.
  • Develop, implement, and monitor individualized case management and treatment plans focused on diversion, re-entry, and relapse prevention. Screening for mental health, suicidal ideation, substance use disorders, and general wellness are expected. Referrals to the LCAS are made in the event of primary substance use concerns.
  • Interfaces directly with collateral providers to confirm treatment, explore potential issues, and gather collateral contact information.
  • Assess clients’ physical and mental wellness, needs, and preferences. Gather collateral material and be part of the intake interview. Assist in arranging evaluations and treatment.
  • Coordinate referrals and facilitate linkage to community-based mental health, substance use treatment, housing, employment, and social services. When working with clients, evaluates family and friend support networks as well as health care access to develop a plan for ensuring community connection.
  • Document interactions and communication with participants. Drafts Monitoring Agreements for review by the Medical Director or designee. Routine review of participants’ progress and compliance with all aspects of Monitoring Agreements.
  • Be available for phone calls from participants for questions. Referrals to community and private-based resources. Facilitate referral to mental health and substance use services in North Carolina and out of state facilities.
  • Provide clinical interventions and counseling as appropriate within the scope of licensure. Serve as an advocate for clients to ensure access to necessary supports and services both during incarceration and post-release.

This position is supported by grant funding for an initial 18 month period, with the opportunity to extend for another 12 months. Following the grant period, there may be opportunities for the role to continue as part of ongoing operations, contingent on program sustainability and organizational needs.

Qualifications:
  • Master's degree in social work, counseling, psychology, or a related field required.
  • LCSW/A, or LCHMC/A OR LMHC, LPC required in the state of NC.
  • Certification in relevant areas such as trauma-informed care or substance use counseling is a plus.
  • A valid drivers’ license and proof of auto insurance is required for this position.
  • Minimum of three years clinical case management experience working with mental health and substance use populations, preferably in criminal justice/diversion or re-entry settings.
