Position Overview

Denali Family Services is seeking a compassionate and dedicated Case Manager to support youth experiencing severe emotional disturbance (SED) and their families through community-based behavioral health services. In this role, you will coordinate support across home, school, and community settings while helping families navigate services, build skills, and access critical resources.

You will work alongside clinicians, educators, and community partners to ensure youth receive the coordinated care and support they need to thrive. Denali Family Services is seeking a compassionate and dedicated Case Manager to support children and families receiving community-based behavioral health services. In this role, you will work directly with youth experiencing severe emotional disturbance (SED) and collaborate with families, schools, and service providers to coordinate individualized supports that promote stability, growth, and long-term success.

Case Managers play a critical role in helping youth and families navigate services, build skills, and access resources within their homes, schools, and communities.

· What You'll Do

· Coordinate services and support for youth and families across home, school, and community settings.

· Develop and monitor individualized service plans and transition plans

· Collaborate with families, schools, clinicians, and community partners to ensure coordinated care

· Assist youth and families in accessing community resources such as education, housing, employment support, and social services

· Facilitate communication among service providers to ensure continuity of care

· Complete timely and accurate documentation in accordance with agency and regulatory standards

· Participate in treatment planning and skill development services as part of the client's care team

· Transport clients to appointments, meetings, or services when necessary

What You Bring

· Bachelor's degree from an accredited college or university

· At least one year of experience providing care coordination or direct services to children and families

· Experience working with individuals identified with developmental disabilities and/or severe emotional disturbance preferred

· Strong communication, documentation, and organizational skills

· Ability to collaborate effectively within a multidisciplinary team environment

Additional Requirements

· Ability to pass the State of Alaska and FBI background checks

· Valid Alaska driver's license with proof of insurance

· Ability to obtain CPR and First Aid certification

· Ability to work in community settings, including homes, schools, and agency locations

Compensation & Classification

This is a full-time salaried non-exempt position, eligible for overtime in accordance with applicable wage and hour laws.

This position is represented by a union and is subject to the terms and conditions of the applicable collective bargaining agreement.
