Case Manager

Job Overview

Reggies Recovery is seeking a compassionate and organized Case Manager to support clients through every stage of the recovery process. The Case Manager plays a critical role in coordinating care, assisting clients with treatment planning, and ensuring individuals receive the resources necessary to maintain long-term sobriety.

This position works closely with therapists, medical staff, behavioral health technicians, and outside providers to help clients successfully transition through detox, residential treatment, and outpatient levels of care.

Responsibilities

• Develop and maintain individualized treatment and discharge plans for clients
• Coordinate services with therapists, medical providers, and treatment staff
• Assist clients with housing, employment resources, and aftercare planning
• Facilitate communication between clients, families, and clinical staff
• Monitor client progress and document case notes in the electronic health record system
• Coordinate appointments for medical, psychiatric, or outside services
• Provide referrals to community resources including sober living, outpatient care, and support groups
• Assist with insurance coordination and continuity of care planning
• Participate in clinical team meetings and case conferences
• Advocate for clients and support their recovery goals

Job Type: Full-time

Pay: $21.50 - $27.00 per hour
