About the Role:


The Case Manager is a great role at Woodland! The Case Manager is responsible for serving as the primary liaison between our adult patients with substance use disorders and outside stakeholders. The Case Manager regularly meets with the patients in their caseload to understand and resolve any barriers to treatment related to family, employment, legal, or financial issues. The Case Manager identifies and evaluates aftercare options to help set patients up for long-term success as well as facilitates and participates in multidisciplinary treatment planning. With a 360-degree view of the patient’s world, the Case Manager is a key advocate, communicator, collaborator, and coordinator.


The Case Manager must develop strong therapeutic relationships, communicate professionally with various stakeholders, maintain meticulous documentation, and remain organized and efficient to have an enduring positive impact on the lives of our patients struggling with addiction.


Minimum Qualifications:



  • Bachelor’s degree in Social Work, Human Services, Psychology, or a related field.

  • Minimum of two years of experience in case management or social services.

  • Strong knowledge of mental health, crisis intervention, and human services systems.

  • Excellent communication and interpersonal skills to effectively engage with diverse client populations.

  • Ability to develop and implement discharge plans and coordinate multi-disciplinary services.


Preferred Qualifications:



  • Master’s degree in Social Work (MSW) or related discipline.

  • Certification in Case Management (CCM) or equivalent credential.

  • Experience working in child welfare or specialized mental health programs.

  • Training in conflict resolution and trauma-informed care.

  • Familiarity with program development and evaluation methodologies.


Responsibilities:



  • Conduct comprehensive assessments of clients’ social, emotional, and health needs to develop individualized care and discharge plans.

  • Coordinate and monitor services by collaborating with healthcare providers, social service agencies, and community resources to ensure clients receive appropriate support.

  • Provide crisis intervention and conflict resolution to assist clients in managing emergencies and overcoming barriers to stability.

  • Maintain detailed and accurate case records, documenting client progress, service delivery, and outcomes in compliance with organizational and regulatory standards.

  • Develop and implement program initiatives aimed at improving service delivery and client outcomes, including child welfare and mental health support programs.


Skills:


The required skills such as case management, social services, and crisis intervention are essential for assessing client needs and coordinating timely support. Mental health expertise and human services knowledge enable the Case Manager to address complex emotional and social challenges effectively. Conflict resolution skills are applied daily to de-escalate crises and facilitate positive outcomes for clients. Developing discharge plans requires organizational and collaborative skills to ensure seamless transitions and continuity of care. Additionally, program development skills support the enhancement of service delivery models, allowing the Case Manager to contribute to systemic improvements and better client experiences.
