Part Time- Contracted Case Manager

Scope of Work Overview

Case Management is the provision and coordination of services that are an integral part of helping individuals access needed medical, social, educational, and other services in order to attain their highest level of independent functioning. Activities include individual’s identification, assessment, reassessment, service planning, referral, service delivery monitoring, and supportive counseling as well as outreach services designed to seek out and engage persons who are eligible for Case Management Services. Individuals seeking mental health services and/or being referred for mental health services must be evaluated for and begin receiving case management services, if they so choose, within fourteen (14) days of the date of the Initial Assessment.

Scope of Work

  • Case Managers will report to the Director and Executive Team
  • Case Managers must be and remain Department of Mental Health Credentialed.
  • Case Managers will keep an accurate account of all Case Management contact through service notes to be kept in clients file.
  • Case Managers primary responsibility is the coordination of services assisting individuals with but not limited to:
  • Medical assistance
  • Social assistance
  • Educational assistance
  • Independent functionality
  • Case Manager is responsible for coordinating introductory and continual services to include but are not limited to:
  • Individuals Identification of mental health status
  • Mental Health Assessment
  • Mental Health Reassessment
  • Service planning
  • Making referrals to external agencies
  • Service delivery monitoring
  • Case Manager is to make contact with every person receiving Case Management services in person periodically.
  • Case Managers will submit a detailed list of individuals enrolled in Case Management services under their caseload and the last date of contact every 30 days to Director and Executive Team.
  • Case Managers will maintain a current, comprehensive file of available community resources that is readily accessible to all Clients. Electronically maintained resource information is permissible. This resource file must include at a minimum:
  • Name of agency
  • Eligibility Requirements
  • Contact person
  • Services available
  • Agencies phone number
  • Case Managers are required to make contact with clients in person periodically. If contact is not possible the client must be properly discharged. Discharge records and attempts of contact must be maintained in clients record.
  • Case Management services must be provided for both adult and youth clients.
  • Case Managers must have reliable transportation, be punctual, and dependable.
  • Case Management services may be executed in office, public school, or other locations.
  • Documentation of services must be submitted in a timely manner in accordance with company policies.
  • Company wide competencies must be completed in a timely manner.
  • Case Manager must attend and complete company trainings and continued educational opportunities.

This part-time contract position allows for a flexible work schedule. Hourly pay will be determined based upon education and experience.

Pay: $22.00 - $35.00 per hour
