Job Summary
We are seeking a dedicated and compassionate Case Manager to join our dynamic behavior health team. In this vital role, you will coordinate patient care plans, facilitate communication among multidisciplinary teams, and ensure seamless transitions across care settings. Your expertise will help optimize patient outcomes, promote efficient utilization of resources, and support patients and their families through complex healthcare journeys. Duties

  • Conduct comprehensive patient assessments to determine individual needs and develop personalized care plans.
  • Coordinate discharge planning, ensuring timely and appropriate transfer of patients to the next level of care or home.
  • Collaborate with healthcare providers, social workers, and family members to facilitate effective communication and care delivery.
  • Review medical records using EMR (Electronic Medical Record) and EHR (Electronic Health Record) systems to support utilization review and management processes.
  • Perform utilization review and management, ensuring compliance with HIPAA regulations and insurance requirements.
  • Assist with intake procedures for new patients, gathering necessary documentation and verifying coverage details.
  • Participate in discharge planning activities to promote safe transitions from program to indipendance.

Requirements

  • Experience with EMR/EHR systems and understanding of HIPAA compliance standards.
  • Knowledge of social work principles, behavioral health considerations, and discharge planning processes.
  • Ability to navigate complex patient cases involving co-occurring disorders.
  • Excellent communication skills for effective collaboration with multidisciplinary teams and patient families.
  • Strong organizational skills with attention to detail in documentation and case management workflows. Pay: $20.00 - $23.00 per hour
