  • Facilitate referrals and warm handoffs to primary care, mental health, SUD treatment, and other community services, and support follow‑through with appointments and care plans.
  • In collaboration with the Program Coordinator, provide or coordinate life skills coaching (budgeting, housekeeping, cooking, transportation planning, communication and conflict resolution) to build resident confidence and independence.
  • In collaboration with the Program Coordinator, provide employment readiness support (résumé development, job search, interview preparation, basic work‑readiness coaching) and coordinate with workforce, training, and social enterprise partners.
  • Encourage and support resident participation in pro‑social, restorative, and faith‑informed activities (support groups, circles, volunteer/service, peer leadership) aligned with Restorative Partners’ mission.
  • Foster a sense of community at the Ranch through resident meetings, restorative dialogues around shared space and safety, and connection to local neighborhood resources.
  • Maintain timely, accurate, and confidential case notes, service plans, and outcome data in case management and/or HMIS systems in alignment with reporting and compliance expectations.
