Position Summary
The Case Manager assesses and coordinates resources to meet the individualized needs of program participants. This role ensures implementation of service and treatment plans, advocates for participants to access necessary services, and identifies program needs and service gaps with recommended solutions. A strong commitment to APIS and RHD values is expected in all duties.
Major Responsibilities
Team-Based Planning

Collaborate as part of a multidisciplinary team delivering holistic, values-based care.

Assist in developing and reviewing recovery plans in partnership with participants, clinical/medical staff, and other stakeholders.

Build and maintain relationships with community agencies to connect participants with appropriate resources.
Direct Client Care
-
Deliver community-based services focused on education, resource connection, community integration, and recovery goal achievement.

Provide services within the community, including transporting participants to appointments as needed.
Administrative Functions

Complete all required assessments and documentation in compliance with organizational, funder, and regulatory standards.

Perform additional duties as assigned.
Qualifications
Education & Experience
-
Bachelor’s degree in Human Services or related field preferred; equivalent experience considered.

Minimum one year of case management experience preferred.

Two years of experience supporting individuals in behavioral health, addiction recovery, or IDD services required.

Valid driver’s license required.

Intermediate computer proficiency.

CPR/First Aid certification preferred.
Requirements

Valid, non-provisional driver’s license and reliable vehicle with required insurance.

Ability to pass required background screenings, including FBI clearance if applicable.
Physical Demands & Work Environment

Ability to lift up to 50 lbs; perform bending, standing, reaching, and driving.

Work conducted in office and community-based settings.

Use of standard office equipment and computer applications.

Adherence to OSHA, HIPAA, safety, and organizational policies.

Participation in required trainings and meetings.
