Job Description:
The Case Manager facilitates communication and program consistency across school, residential, and home settings to support each student’s social and academic success. Serving as a liaison among team members, the Case Manager ensures the effective flow of information and provides counseling services, as needed, in accordance with the student’s IEP.
Essential Duties:
- Collaborate with staff to evaluate each student’s social-emotional functioning as it affects their availability for learning within the educational and residential settings, measuring annually or as needed.
- Facilitate/coordinate visits from parents, social service workers, legal teams, educators, and other community professionals or consultants.
- Working with other team members, provide guidance to staff on a variety of topics, including effective approaches to working with special needs students, classroom management techniques, teaching and learning strategies and crisis management.
- Collaborate with families and community-based agencies to provide a comprehensive model of school wide educational and mental health services
- Participate in multidisciplinary teams and meetings (with in house and community consultants, Facilitate Student Status meetings, etc., virtual appointments).
Job Qualifications:
- Must have a Bachelor’s degree in Counseling, Social Work or related field, Masters preferred.
- Five years’ experience working with individuals with intellectual disabilities, multiple disabilities and/or autism preferred.
- Must possess excellent communication skills, professionalism and leadership skills
- Must be a minimum of 21 years old
- Must provide proof of authorization to work in the U.S.
- Must pass pre-employment drug screening, criminal background checks (CJIS/Family Law Article 5-561), and CPS clearance.
- If driving is required, must possess a valid driver's license in good standing and meet insurability requirements.
- Must be able to perform Safety-Care physical interventions, except for approved temporary or minimal restrictions authorized by the Behavior Department.

Pay: $53,000.00 - $71,000.00 per year
