Mercy House, a 24-hour emergency shelter serving individuals experiencing homelessness, is seeking a compassionate, motivated, and resourceful Case Manager to join our team. What You'll Do
As a Case Manager, you'll work directly with shelter guests to help them identify goals, navigate challenges, and connect with life-changing resources. Using a trauma-informed and person-centered approach, you'll empower individuals to move forward on their journey toward self-sufficiency.
Your days may include:
- Building positive relationships with guests and helping them identify and achieve personal goals
- Conducting assessments, intakes, and individualized service planning
- Assisting guests with housing searches, public benefits, and community resource connections
- Coordinating referrals to healthcare, behavioral health, legal, employment, and social service providers
- Providing crisis intervention, advocacy, encouragement, and ongoing support
- Maintaining accurate documentation and case notes in HMIS and other required systems
- Collaborating with community partners and internal teams to ensure guests receive comprehensive support
- Helping create a welcoming, respectful, and safe shelter environment
- Assisting with daily shelter operations and guest support as needed

Minimum Qualifications
- High School Diploma or GED
- At least one year of experience working with vulnerable populations in a case management, direct service, or related role
- Valid New York State Driver's License
- Ability and willingness to obtain CPR/First Aid and Narcan certification
Preferred Qualifications
- Knowledge of homelessness, mental health, substance use, and community resources
- Experience with crisis intervention and trauma-informed care
- Familiarity with Housing First principles and public benefit systems
- Associate's or Bachelor's degree in Human Services or a related field
- Experience using HMIS or other case management software
