Job Overview
We are seeking a dedicated and compassionate Case Manager and Substance Abuse Counselor to join our dynamic behavioral health team. Duties
- Conduct thorough patient assessments to determine individual needs, including diagnostic evaluation and addiction counseling strategies
- Develop personalized treatment plans incorporating psychotherapy, behavioral therapy, cognitive behavioral therapy (CBT), and motivational interviewing techniques
- Provide individual and group counseling sessions focused on substance abuse recovery, mental health stabilization, and behavior management
- Facilitate crisis intervention and crisis management to ensure client safety during acute episodes or emotional distress
- Coordinate care with families and community resources, including child & family counseling and home & community care services
- Maintain accurate medical documentation, records, and notes in compliance with HIPAA regulations using ICD-9/ICD-10 coding standards
- Monitor patient progress through data collection, patient care assessments, and ongoing clinical counseling to adapt treatment plans as needed
Skills
- Expertise in psychotherapy modalities such as behavioral therapy, cognitive behavioral therapy (CBT), and applied behavior analysis (ABA)
- Proficiency in motivational interviewing techniques to foster client engagement and motivation for change
- Strong background in crisis intervention, crisis management, and trauma-informed care including PTSD care strategies
- Experience with medical terminology, medical records management, intake procedures, and HIPAA compliance
- Knowledge of addiction medicine, mental health counseling, diagnostic evaluation processes, and case management practices
- Ability to conduct patient assessments using ICD coding systems (ICD-9/ICD-10) for accurate diagnosis documentation
- Skilled in group therapy facilitation, social work principles, behavior management strategies, and data collection for clinical research purposes
