Position: Case Manager

Regional Health Systems is Northwest Indiana’s largest provider of community-based mental health care, primary health care and addiction treatment services. We offer adults and children—both uninsured and insured—integrated care through our community mental health center and federally qualified health center. Regional Health Systems is a member of the Regional Care Group.

Position Purpose:

The Intake Case Manager completes risk assessments, DARMHA documentation, screening tools treatment planning, and assists with safety planning. They assist in identifying the appropriate level of care. The role includes providing orientation to services, coordinating referrals and scheduling and ensuring timely, accurate documentation in compliance with agency and regulatory standards.

Location:

• Assisted Outpatient Treatment Department – Merrillville, IN

Requirements:

• Bachelor’s degree in social work, Psychology, or related clinical field
• One year of experience with the target population (adult & mental health)
• Driver’s License & Insurance and Clean Driving Record

Shift Details: Full-time – Non-exempt: Days and evenings depending on the needs of the department, and may include some weekends.
