The Case Manager provides support and care coordination, considered essential to assist individuals who have SPMI diagnoses with improving functioning, gaining access to necessary services, and creating an environment that promotes recovery as identified in the Individual Recovery Plan (IRP). The focus of interventions includes assisting the individual with 1) developing natural supports to promote community integration; 2) identifying service needs; 3) referring and linking to services and resources identified through the service planning process; 4) coordinating services identified on the IRP to maximize service integration and minimize service gaps; and 5) ensuring continued adequacy of the IRP to meet individuals’ ongoing and changing needs.


Flexibility in scheduled hours is necessary to provide effective non-clinic/community-based services throughout the day and some evening hours, as needed. This position also requires collaboration with a team of professional staff to assess needs, develop treatment plans, and monitor individuals’ progress.


This position requires the ability to work in a fast-paced setting, a passion to work with those with Behavioral Health needs, families, and the ability to work closely and effectively with a multidisciplinary team. Reliable transportation and driving abilities are imperative.


Duties & Responsibilities:

  • Engages individuals, family and/or support system(s) to assess level of functioning, current Strengths, Needs, Abilities, and Preferences and secure information relative to program requirements for receiving services and recommending appropriate level of care.
  • Engage individuals in a recovery-based partnership. Assists individuals with developing a community-based support network to facilitate community integration and maintain housing stability.
  • Provides Case Management (CM), Skills Training (PSR-I) service in accordance with DBHDD provider Manual guidelines – see Required Components & Staffing sections.
  • Coordinates care activities and assist the individual as they move between and among services and supports.
  • Assists the individual with referral and linkage to services and resources identified on the IRP including housing, social support, family/natural supports, entitlements (SSI/SSDI, Food Stamps, VA), income, transportation, etc.
  • Visits the individual in the community to jointly review progress made toward achievement of IRP goals and to seek input regarding level of satisfaction with treatment and any recommendations for change.
  • Participate in monthly treatment team meetings to assist in the development of IRP/treatment plans with appropriate therapeutic interventions, notify professionals of any unusual observations or incidents, progress, and follow up.
  • Enters accurate and complete documentation (within 24 hours) to meet agency and state standards and provide effective service delivery across a broad range of services.· May provide individual skills training as indicated in the client’s treatment plan including but not limited to independent living skills, coping skills, interview skills, symptom management skills, anger management skills, assertiveness skills, advocacy skills, hygiene skills, and many other daily living skills.
  • Recognize the need for crisis intervention and work with licensed professionals to assess higher need of care Notifies security, supervisor, and/or professional staff of escalated situations.
  • Communicate and work closely with family members to support individuals in crisis. May assist with coordination of transportation, completion of all required documentation and referrals for admission to the CSUs and local hospitals. Assist with hospital discharge planning as needed.
  • Provides strength-based interventions and developing skills that are directly associated with the client’s individual target behavior and related back to their treatment plan.
  • Maintain update authorizations for billing and notify clinicians when authorizations are needed.
  • Maintain accurate mileage log and entries to daily timesheet.
  • Maintain updated list of community resources.
  • Other related duties as assigned.


Minimum Qualifications:

  • High School Diploma or GED AND three – (3) years of job related experience in a social services-related position OR one-(1) year of experience at the lower level Social Services Tech 2 (SST011) or position equivalent.

Preferred Qualifications:

  • Superior verbal and written communication skills.
  • Ability to multi-task and manage time effectively.


Requirements/Competencies:

  • Must be able to lift 20 pounds.
  • Requires long periods of sitting/standing.
  • Must be able to respond quickly to sounds.
  • While performing the duties of this job, the employee is frequently required to sit, move about, hear and speak.
  • Candidates for selection must pass a criminal background check (including fingerprinting)
  • Must pass a pre-employment drug screen and subject to random drug screens.
  • Any combination of training and experience, which would have enabled the applicant to acquire the necessary knowledge, skills and abilities.

Note: Some positions may require a valid driver's license.
