**Job Overview:**

We are seeking dedicated Qualified Mental Health Professionals (QMHP) with community based experience to join our team. The ideal candidate will assist in providing care and support to individuals experiencing mental health challenges. This role involves working closely with patients, clinical staff, and other healthcare professionals. Experience with discharge planning is a plus. This position will start as a FT position. Chosen candidate will be benefit eligible after FT status is achieved and the successful completion of a 90 day probationary period.

**Responsibilities:**

- Conduct patient assessments to evaluate mental health needs

- Provide mental health counseling and support to patients

- Assist in developing and implementing treatment plans

- Collaborate with clinical staff on patient care and progress

- Maintain accurate medical documentation and records

- Participate in case management meetings

- Experience with ANSA and CANs assessments

**Requirements:**

- Previous experience in community based mental health services

- Knowledge of clinical counseling techniques and patient assessment

- Understanding of outpatient mental health services

- Proficiency in medical documentation and record-keeping

- Ability to work collaboratively in a multidisciplinary team

- Strong interpersonal and communication skills
