Case Manager-Day Habilitation

This Case Manager-Day Habilitation is based out of the Albany office, and involves floating around the capital region.

The Case Manager-Day Habilitation is responsible for the development and review for all Day Habilitation Plans and “Outcomes” at our Day Community Opportunities Program.

The Case Manager will also ensure “Outcomes” are being carried out according to the Day Habilitation Plan and will act as an advocate for all individuals enrolled.

Job Responsibilities:

Participant Planning & Clinical Oversight

  • Develop, update, and oversee individualized Day Habilitation/Staff Action Plans at least semi-annually.
  • Coordinate plan reviews with participants, families, care managers, and clinical teams to assess progress and address needs.
  • Monitor participant satisfaction, advocate for participant needs, and support informed choice, problem-solving, and valued outcomes.
  • Ensure timely follow-up on participant and family concerns.
  • Maintain accurate participant records to ensure compliance and audit readiness.

Documentation, Billing & Netsmart Oversight

  • Ensure all required participant documentation is completed and uploaded in Netsmart in a timely manner.
  • Oversee completion and approval of electronic Daily Service Records (DSRs).
  • Complete weekly billing and coordinate with county management to ensure documentation accuracy and compliance.

Staff Training & Support

  • Provide clinical guidance and oversight to Day Opportunity Specialists (DOS).
  • Train staff on incident reporting, problem resolution, documentation standards, and Netsmart processes.
  • Educate participants on grievance procedures and individual rights.

Care Management & Community Liaison

  • Serve as a liaison between Day Habilitation staff and Care Managers.
  • Facilitate communication, mediate conflicts, and support resolution of issues involving families and advocates.
  • Participate in community outreach, intake meetings, and networking to support program growth.

Program Administration & Leadership

  • Participate in meetings, committees, and cross-functional initiatives.
  • Support program scheduling, oversight, and staff supervision as needed.
  • Assist with intake coordination, transition planning, and ensuring all intake documentation is complete prior to billing.
  • Collaborate with leadership to create engaging, meaningful program schedules.

Additional Responsibilities

  • Provide direct care support in the field when needed.
  • Offer general program support and assist with operational needs as they arise.
Requirements:
  • Bachelor’s Degree in human services and one (1) year of experience in providing services to persons with developmental disabilities.
  • Good writing, typing and interpersonal communication skills.
  • Valid NYS driver’s license and reliable transportation.
