Case Manager for adults with disabilities

Progressive Community Services is hiring an IRIS Consultant (IC) to provide case management to adults with disabilities who want to self-direct their long term care supports. This is a home-based, full-time position with excellent salary and benefits. Must have a Bachelor’s degree in social work or related field, Master’s degree preferred. Bilingual speakers are encouraged to apply.

PCS IRIS Consultants must live within 30 minutes of Dane, Dodge, Jefferson, Columbia, Marquette or Rock County, Wisconsin.

Responsibilities
-Complete monthly phone calls and quarterly visits with caseload.

- Conduct comprehensive assessments of clients' needs and develop individualized support plans
- Maintain accurate documentation and comply with regulatory requirements.
