Case Manager for Senior Care

Job Description: Care Choice Home Care is seeking a highly qualified and dedicated Case Worker with experience in social work and a variety of population. This position involves coordinating care for clients by assessing their needs, developing personalized care plans, and managing service delivery. This position requires commuting between Orange County and San Diego County. Providing community resources and a support within the community.

Responsibilities:

- conduct biopsychosocial assessments to determine clients' needs.

- develop, coordinate, and monitor personalized care plans in collaboration with healthcare professionals.

- advocate for clients to access necessary services and support.

- evaluate the effectiveness of care plans and make adjustments as necessary.

- manage communication between clients, families, and healthcare providers.

- provide training and support to other staff members on client-specific needs.

Work environment:

- position will involve working in various settings, including healthcare facilities, community agencies, and directly in clients’ homes.

- collaboration with government workers as the position involves interfacing with state and local government agencies.

Professional skills:

- expertise in care management.

- strong organizational and communication skills.

- proficiency in understanding and applying medicare regulations and insurance requirements.

- ability to handle high-stress situations including emergency and crisis scenarios.

Qualifications:

- valid driver's license and a clean driving record.

Salary: competitive; commensurate with experience and qualifications. salary range typically falls between $38,000 and $50,000 depending on experience, geographic region, and work setting.
