Case Manager (Full Time)

Job Responsibilities:

· Takes ownership of treatment access and continuity by actively removing real-world barriers such as housing, transportation, employment, legal obligations, benefits, and discharge planning

· Weekly 30-minute client contact required to include family/referral source outreach.

· Manage up to 30 active patients in PHP

· Documents clearly to support stability and long-term recovery success.

· This role ensures that treatment plans are actively advanced rather than maintained.

· This role intervenes when risk to recovery emerges from outside factors.

· Knowledge of common psychiatric disorders and/or dual diagnosis and various treatment modalities of the population served preferred.

· CPR and de-escalation certification required (training available upon hire and offered by facility).

· First aid may be required based on state or facility requirements.

· Document all interactions accurately and timely in compliance with CARF and VA Code

· Maintain confidentiality of all client records and information

· Participate in staff development, quality improvement, and training sessions

· Perform general office duties and support facility operations

Working Conditions:

The Case Managerwill work primarily in an office setting with regular interaction with clients and their families. The Case Managermay be required to work evenings or weekends as needed to accommodate client needs.
