Case Manager – HEART Family Strengthening Program

(CBCI) is a nonprofit human services organization founded upon 20 years of expertise in delivering positive youth development, family strengthening, and community empowerment services for hard-to-serve populations in Broward and Palm Beach Counties. Our mission is to expand the available options for helping children to be socially and academically successful despite the many challenges they face and regardless of the many negative influences around them.

The Case Manager plays a vital role in the HEART (Helping Every Area of Relationships Thrive) Family Strengthening Program at CBCI. This position is responsible for providing trauma-informed, individualized support to program participants through the development and monitoring of Individual Development Plans (IDPs), referrals to services, and consistent case management. Case Managers are central to participant retention, engagement, and achievement of goals related to healthy relationships, parenting, and economic stability.

The Case Manager Key Responsibilities:

  • Will conduct comprehensive intake sessions, assessments, and screenings, including domestic violence screening and family functioning tools.
  • Collaborate with participants to develop personalized Individual Development Plans (IDPs) that reflect goals related to relationship skills, parenting, employment, and wellbeing.
  • Maintain regular contact with assigned participants through in-person meetings, phone calls, and virtual check-ins to monitor progress and offer coaching and encouragement.
  • Refer participants to appropriate wraparound services, including mental health counseling, substance abuse support, housing, childcare, employment training, and financial assistance.
  • Track attendance, progress, and engagement in the nFORM system and maintain accurate, up-to-date case files in compliance with program and federal standards.
  • Participate in team meetings, case reviews, and supervision to ensure coordination across program components and alignment with best practices.
  • Support workshop engagement by monitoring participant needs, encouraging attendance, and helping troubleshoot barriers to participation.
  • Assist with evaluation efforts by administering participant feedback tools and collaborating with the data and evaluation team.
  • Ensure participant confidentiality and adherence to all privacy and mandated reporting policies.

Qualifications:

  • Bachelor’s degree in social work, psychology, human services, or a related field required; equivalent experience may be considered.
  • At least 2–3 years of direct case management experience, preferably in family services, workforce development, or community-based programs.

Pay: $45,000.00 - $48,000.00 per year
