Community-Based Connections, Inc. (CBCI) is a nonprofit human services organization founded upon 20 years of expertise in delivering positive youth development, family strengthening, and community empowerment services for hard-to-serve populations in Broward and Palm Beach Counties. Our mission is to expand the available options for helping children to be socially and academically successful despite the many challenges they face and regardless of the many negative influences around them.
The Case Manager plays a vital role in the HEART (Helping Every Area of Relationships Thrive) Family Strengthening Program at CBCI. This position is responsible for providing trauma-informed, individualized support to program participants through the development and monitoring of Individual Development Plans (IDPs), referrals to services, and consistent case management. Case Managers are central to participant retention, engagement, and achievement of goals related to healthy relationships, parenting, and economic stability.
