Case Manager, Home Care

<Duties>

  • Conduct comprehensive patient assessments to determine individual needs, medical status, and care plans.
  • Develop personalized discharge planning strategies that facilitate smooth transitions from inpatient or outpatient settings to home care.
  • Coordinate with healthcare providers, social workers, and family members to ensure continuity of care and adherence to treatment plans.
  • Manage utilization review and medical records documentation in compliance with HIPAA regulations.
  • Facilitate intake processes and evaluate diagnostic information to support appropriate care delivery.
  • Monitor patient progress through regular follow-ups, adjusting care plans as needed to meet evolving health requirements.
  • Educate patients and caregivers on medication management, safety protocols, and available community resources to promote optimal recovery.

<Requirements>

  • Strong background in acute care, inpatient or outpatient settings, with familiarity in Home Health Care.
  • Knowledge of physiology, medical terminology, discharge planning, and diagnostic evaluation techniques.
  • Experience with case management practices including utilization management and post-acute care coordination.
  • Excellent patient service skills with the ability to perform thorough patient assessments and develop effective care strategies.
  • Familiarity with managed care processes, medical records management, and HIPAA compliance standards.
  • Ability to work independently while collaborating effectively within multidisciplinary teams across hospital medicine and clinic environments.
