JOB SUMMARY
HealthRIGHT 360, a nonprofit organization and a family of programs, is committed to providing accessible and comprehensive healthcare services to vulnerable populations. Our mission is to tackle systemic barriers to healthcare and promote health equity for all. We offer a wide range of services, including mental health care, residential and outpatient substance use treatment, and primary health services. Additionally, we provide transitional support for individuals re-entering the community after involvement in the criminal justice system. By integrating physical and behavioral health, we empower individuals to overcome challenges by addressing social determinants of health, fostering resilience, and facilitating recovery.

The African American Healing Center is working with clients to prevent and reduce violence in our communities. Services are designed to meet the needs of people with all backgrounds, while proactively serving communities of color with programs that honor African American history and that are sensitive to the cultural context of each client.

The Case Manager I is responsible for assessing treatment needs; monitoring and evaluating problem lists and progress and facilitating interdisciplinary approaches. As the Case Manager I coordinate an approach to the delivery of health, substance use, mental health, and social services, linking clients with appropriate services to address specific needs and achieve stated goals. The Case Manager I’s duties include care coordination, maintaining positive working relationships with outside community partners, scheduling appointments, arranging transportation, assisting with acquiring medications, ongoing care and discharge planning, and crisis intervention.

KEY RESPONSIBILITIES
  • Work as an active team member and closely collaborate with team members.
  • Help clients get and stay engaged in SUD treatment.
  • Help clients transition between levels of care.
  • Assess for opioid overdose and ensure beneficiary has Naloxone if appropriate.
  • Interface with a multidisciplinary team in treatment planning and service delivery.
  • Develop and assess the effectiveness of individualized treatment and consumer progress.
  • Maintain documentation in compliance with agency, HIPAA, and DPH standards.
  • Assist in the ongoing maintenance of consumers’ charts and other related documentation.
  • Perform other duties as assigned by the Program Manager.
  • Arrange the work schedule in accordance with the agency’s needs.
  • Comply with the agency’s policies and procedures.
  • Attending internal and external meetings as assigned.
  • Create a safe space for clients and maintain appropriate boundaries.
  • Meet expected performance standards as assigned by the supervisor.
  • Provide “whatever it takes” services to consumers including providing services where the consumer lives, assisting with housing, benefits, employment, education, transportation, childcare, medical, and other needed services.
  • Provide case management services to clients as assigned.
  • Serve as a care coordinator for clients as assigned. Responsibilities include coordination of all services with other providers and completion of all coordinated care documentation.
  • Provide case management services to clients as assigned.
  • Assist the client in developing independent living skills to promote independence and self-sufficiency.
  • Assist in crisis and symptom management.
  • Acts as a buddy to new hires.
And perform other duties as assigned.

QUALIFICATIONS
Education, Certification, and Experience
  • High school diploma or GED required.
  • Valid CA Driver’s License.
  • First Aid and CPR Certification.
  • AOD Registration preferred.
  • Experience in case management required.
  • Experience working with the criminal justice population preferred.
Knowledge
  • Some experience providing case management services is required.
  • Culturally competent and able to work with a diverse population.
  • Knowledge of housing, employment, SSI resources, and success with linking clients.
  • Experience working successfully with issues of substance abuse, mental health, criminal background, and other potential barriers to economic self-sufficiency.
  • Knowledge of co-occurring disorders and trauma-informed treatment preferred.
