JOB SUMMARY
HealthRIGHT 360, a nonprofit organization and a family of programs, is committed to providing accessible and comprehensive healthcare services to vulnerable populations. Our mission is to tackle systemic barriers to healthcare and promote health equity for all. We offer a wide range of services, including mental health care, residential and outpatient substance use treatment, and primary health services. Additionally, we provide transitional support for individuals re-entering the community after involvement in the criminal justice system. By integrating physical and behavioral health, we empower individuals to overcome challenges by addressing social determinants of health, fostering resilience, and facilitating recovery.
PRCS, in collaboration with the Los Angeles County Probation Department, offers a range of services to individuals across the county, facilitated through various housing facilities including Transitional Housing, Reentry Housing, and specialized accommodations for parents with children, board and care, among others. Our reentry planning team involves Case Managers, System Navigators, and Service Coordinators. Our team provides support services including job training and permanent housing move-in costs, facilitated through Probation Department referrals. Our community-based programs foster partnerships with local health providers, housing advocates, employment resources like trade unions’ apprenticeships, and human services agencies. This comprehensive approach aims to meet the diverse needs of participants and their families, fostering resilience and reducing recidivism risks.
The Case Manager I is responsible for working in conjunction with subcontracted Community-Based Transitional, Sober Living, and Employment Services Providers (CBPs) to assess client needs and strengths in support of developing and securing permanent, independent, or supportive housing, employment, auxiliary funds, and ancillary resources. Through an array of case management services and by providing high-quality customer service, the Case Manager I collaborates with CBPs to match clients with appropriate available permanent housing and employment opportunities, tracking outcomes for each of them. The Case Manager I is also responsible for maintaining up-to-date documentation for each client. Coordinates with HealthRIGHT360 management to facilitate auxiliary services that address participants’ holistic needs and support successful reentry outcomes, including transportation and the documentation of auxiliary items provided to participants.
KEY RESPONSIBILITIES
Case Management and Client Support
- Carry caseload of up to 60 clients.
- Review client information with Community-Based Providers (CBP) and coordinate client
placements into housing and employment resources.
- Coordinate site visits for current and prospective CBPs and document findings.
- Collaborate on placement of clients into post-transitional housing and employment resources.
- Monitor and track CBP services delivery for quality assurance.
- Deliver services in the field.
- Transport clients as needed.
Documentation and Reporting
- Maintain accurate records by entering data into an electronic database.
- Maintain accurate and up-to-date files for clients, including probation-required documents.
- Document client status and progress for permanent housing and job placement opportunities.
- Screen project documents and records for quality assurance and improvement reports.
- Track outcomes and monitor subcontractor contractual and service provision quality assurance.
- Identify and track all clients’ outcomes for reporting to County PRCS representatives.
- Ensure compliance with guidelines established by HealthRIGHT360 for internal and external
evaluations.
Administration and Other Duties
- Attend and actively participate in assigned supervision.
- Participate in agency, team, and assigned meetings.
- Participate in training opportunities and complete assigned training.
- Arrange work schedule according to program needs.
- Read and respond to emails when appropriate.
And perform other duties as assigned.
QUALIFICATIONS
Credentials
- High school diploma or equivalent plus two years of case management experience or Bachelor’s
degree in human services field.
- Valid First Aid and CPR certification or ability to obtain within 30 days of hire.
- Valid California Driver’s License and access to registered and insured transportation.
Experience
- Two years’ case management experience or behavioral health work experience preferred.
- Experience in the human service field and experience in substance abuse treatment, relapse
prevention, and recovery.
