Case Manager IDD Waiver

• Full-time Case Manager is needed for our Parkersburg, WV office.
• A Bachelor’s degree in the Human Service Field is required
• Work is with children and adults within the WV IDD Waiver Program
• Participate in Annual Assessments
• Develop individual service plans
• Facilitate IDT meetings
• Linkage and Referral
• Advocacy
• Crisis Response Planning
• Documentation of case management activities
• Travel to client homes for monthly visits, meetings and assessments
