SUMMARY: Under the supervision of the Clinical Director or designated supervisor, the Case Manager III is responsible for all case management activities associated with the designated program or programs assigned.
Requirements:Essential Duties and Responsibilities
•Provide back up for other staff as needed.•Participate in Peer review chart audits
•Conduct intake assessments for new clients and ensure all required documentation is completed and submitted in CHAMP/HMIS. •Coach department Staff, Case Managers I & II.

Education: High School Diploma with 6 years of experience or a bachelor’s degree with 3 years of experience or a Master’s degree in Social Work or related field with internship plus six or more months experience in Case management.
Language Ability: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals.
Math Ability: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
Reasoning Ability: Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
Computer Skills:
To perform this job successfully, an individual must have very good knowledge of Word processing software and Excel spreadsheet software.
