Case Manager in Dane County

The Community Action Coalition for South Central Wisconsin is a tri-county non-profit organization serving the counties of Dane, Jefferson and Waukesha that works in a coordinated and comprehensive manner to develop and implement community-driven programs and services that will make a critical difference in the lives of low-income families and individuals, that will advance the revitalization of low-income communities, and that will ultimately provide empowerment of low-income families and individuals in rural and urban areas.

The Case Manager provides direct support to individuals and families seeking assistance through Community Action Coalition programs. This role focuses on intake, assessment, service planning, and connecting participants with community resources. Case Manager I is an entry to mid-level position designed for individuals who are developing their professional expertise in client services.
