About Cleveland Rape Crisis Center
Cleveland Rape Crisis Center (CRCC) supports survivors of rape and sexual abuse, promotes healing and prevention and advocates for social change. CRCC was founded in 1974 in response to Cleveland’s long-standing need for services to assist survivors of sexual assault.
CRCC offers services in Cuyahoga, Ashtabula, Geauga and Lake counties such as legal advocacy, case management, and therapy services. We also do education and outreach work, to educate the community on sexual violence and human trafficking prevention.
Position Summary
The Case Manager JJS is responsible for providing effective linkage, collaboration, and referrals to assist clients in their healing process and to reduce barriers to services. In line with CRCC’s coordinated care model, the Case Manager JJS assures that clients receive timely access to necessary services and assists with daily needs and goal planning. The Case Manager JJS meets clients where they are and is creative and open-minded about how to best assist clients in meeting their needs. The Case Manager JJS utilizes a strength-based and trauma-informed approach to client care with survivors of human trafficking and sexual violence.
Key Responsibilities
- Maintain individual caseload and assist clients whose lives have been disrupted due to sexual violence to regain independence and stability
- Adhere to predetermined monthly productivity requirements
- Refer and link survivors of sexual violence and their families to appropriate community agencies such as legal services; long-term counseling; and for medical, mental, physical, safe housing, social and educational needs
- Coordinate with other CRCC team members for service coordination and to assure timely services for all survivors
- Provides psychoeducational groups to clients of all ages and their families in the community
- Monitor client’s progress through the creation and evaluation of individual service plans (ISPs)
- Monitor cases by verifying clients' attendance, observing and evaluating engagement and responses, advocating for needed services, obtaining additional resources, intervening in crises, and providing personal support
- Maintain clients' records and event records by reviewing case notes, logging events and progress
- Communicate clients' progress in interdisciplinary meetings and evaluations; disseminating results and obstacles to progress
- Organize and facilitate psychoeducational groups for girls involved with the Court and their parents/caregivers, as appropriate (S.E.L.F. Curriculum through the Sanctuary Institute).
- Reach girls involved with the juvenile justice system with a vital evidence-based prevention curriculum (Girls Circle).
- Travel to other organizations and throughout the community to meet clients offsite
Education, Experience, Licenses, & Certifications
- Associate’s in Human Services, Social Services; Bachelor’s preferred
- 2 years of experience in case management working with clients with complex mental health needs or impacted by sexual violence
- Valid OH license: Licensed Social Worker (LSW) preferred
- Valid driver’s license and reliable transportation
- May consider an equivalent combination of education, experience, and/or training to meet the essential functions of the position
Salary & Benefits
Salary Range: $40,000 - $47,000 annually
