Case Manager-Mental Health/Substance Abuse Facility

Job Summary: The Case Manager will play a crucial role in coordinating and providing support for clients with mental health and/or substance use disorders. This position involves assessing client needs, developing and monitoring individualized treatment plans, and connecting clients with appropriate resources. The Case Manager works closely with a multidisciplinary team to ensure that clients receive the care and support they need for recovery and well-being.

Key Responsibilities:

  • Assess Client Needs: Conduct intake assessments and interviews to gather comprehensive information about the client's mental health, substance use history, and personal circumstances.
  • Develop and Implement Treatment Plans: Work collaboratively with clients and the treatment team to create individualized treatment plans tailored to each client’s needs, goals, and progress.
  • Monitor Client Progress: Regularly follow up with clients to evaluate their progress toward treatment goals, adjusting plans as needed and addressing any challenges.
  • Provide Support and Advocacy: Act as the primary point of contact for clients, offering emotional support and advocacy to ensure they understand and follow their treatment plans.
  • Coordinate Services: Link clients to community resources and external services such as housing, medical care, social services, legal support, and vocational training.
  • Document and Record Keeping: Maintain accurate, up-to-date documentation of client interactions, progress, and any changes in treatment plans or needs, ensuring compliance with facility policies and HIPAA regulations.
  • Crisis Intervention: Respond to crisis situations, providing immediate support and guidance, and collaborating with clinical staff and emergency services when necessary.
  • Collaborate with Team: Work with a multidisciplinary team, including therapists, physicians, and other behavioral health professionals, to coordinate comprehensive care and ensure holistic treatment.
  • Educate Clients: Provide education on mental health and substance use disorders, recovery strategies, and coping mechanisms to empower clients in their recovery journey.
  • Ensure Compliance: Ensure clients are following treatment protocols, attending necessary appointments, and adhering to the facility’s policies and guidelines.
  • Case Documentation and Reporting: Maintain timely and accurate documentation for case management activities, client progress, and treatment goals to ensure the effectiveness of interventions and compliance with regulations.

Qualifications:

  • Bachelor’s degree in social work, psychology, counseling, or a related field required
  • Previous experience in a case management role in a mental health, substance abuse, or social services setting is highly preferred.
  • Knowledge of mental health and substance use disorders, including common treatments and recovery strategies.
  • Strong understanding of community resources and social services available for clients in need.
  • Excellent communication, organizational, and problem-solving skills.
  • Ability to work effectively with individuals from diverse backgrounds and those in recovery.
  • Compassionate and empathetic approach to client care.
  • Ability to handle crisis situations with patience and professionalism.
  • Must be able to work both independently and as part of a team.
  • Valid Ohio driver’s license and reliable transportation (if applicable for outreach services).
  • CPR and First Aid certification preferred (or willingness to obtain).

Physical Requirements:

  • Ability to sit or stand for long periods.
  • Ability to lift and carry items up to [specified weight limit] as needed.
  • Must be able to travel within the community or to other facilities as needed.

Job Type: Full-time

Pay: $40,000.00 - $50,000.00 per year
