Case Manager (Ombudsman Program)

POSITION SUMMARY

We are pleased to offer the opportunity to work as a Case Manager within the DuPage County Department of Community Services. Case Managers provide comprehensive case management and/or service coordination in a variety of areas. The current Case Manager opening is in the following programs/assignment area:

Long Term Care Ombudsman-
Case managers in this program represent and advocate for residents, age 60 and older, in long term care facilities, licensed assisted living and supportive living facilities. They receive, investigate, and attempt to resolve concerns and complaints regarding the care of residents as well as providing a regular presence in the licensed facilities.

This is a grant funded position.

RESPONSIBILITIES

Responsibilities Include:
  • Identify, investigate, and seek to resolve complaints on behalf of long-term care residents
  • Provide information and assistance, and community education
  • Maintain a regular presence in long-term care settings
  • Support and assist in the development of resident and family councils in LTC settings
  • Provides follow-up and monitoring
  • Completes all mandated paperwork with accuracy and in a timely manner
  • Utilizes various computer programs for daily tasks and reports
  • Performs other related duties as required

REQUIREMENTS INCLUDE

Requirements include the following experience or equivalent combination of training and experience:
  • Completion of a B.A. or a B.S. Degree in Social Science, Human Services, Social Work, or a related field or be a RN or LPN
  • One (1) year of experience in assessment or service provision
  • Must possess and maintain in good standing, a valid Illinois Driver’s License
The preferred candidate will:

  • Be fluent in a foreign language

Travel by automobile in all weather conditions to varied residential and on-site environments based on assignment. May include exposure to noxious odors, hoarding, unsanitary conditions and other related conditions and situations. The noise level in the work environment is usually moderate.
