The New Covenant Center, a program of Catholic Charities located in Stamford, CT, is seeking a compassionate part-time (20 hours/week) Case Manager to support individuals and families facing crisis. Reporting the Senior Case Manager, the part-time Case Manager is responsible for providing services focusing on crisis intervention and advocacy for individuals and families facing mental/behavioral health issues. Responsibilities include resolution of issues relating to, but limited to, housing, unemployment, SNAP/State benefits, and other immediate needs. The Case Manager, under the direction of the Senior Case Manager, will work together to create a plan for long/short-term self-sufficiency. This position also provides supervision of the Café floor during meals.
ACCOUNTABILITIES:
- Complete initial intake interviews with new clients and conduct thorough needs assessments to determine program eligibility, needs, eligibility, strengths, and barriers.
- Work with clients to establish service plans leading to crisis resolution and self-sufficiency.
- Connect clients with internal programs and external community resources (e.g., mental health services, financial assistance, housing support).
- Track client progress and prepare required reports while maintaining accurate, timely case notes, assessments, and documentation in accordance with program policies and funding requirements.
- Perform market and best practices analysis of case management industry.
- Build and maintain collaborative relationships with local agencies, service providers, and community partners while developing a robust pipeline of resources and contacts within Fairfield County to support client access to services.
- Cross training within all Operations areas of organization.
- Supervise and monitor daily activities of dining room during mealtimes.
- Perform other duties as assigned.
ESSENTIAL EDUCATION, SKILLS, QUALIFICATIONS:
Education and Experience:
- High school diploma, two‑year college degree, or equivalent relevant work experience, with at least two years of experience in case management, assessment, counseling, or crisis intervention preferred.
- Strong verbal, written, negotiation, and interpersonal skills, with the ability to adapt to frequently changing tasks and initiatives.
- Highly organized and detail‑oriented, demonstrating effective time‑management, reliability, initiative, and the ability to manage multiple tasks in a fast‑paced environment.
- Proficient with computers, particularly Office 365, and familiar with the 2‑1‑1 database.
- Bilingual English/Spanish conversational skills strongly preferred.
Skills and Abilities:
- Demonstrates excellent communication, sound judgment, and strict adherence to privacy and confidentiality standards.
- Builds rapport effectively with diverse populations and collaborates well within team environments, showing strong interpersonal and teamwork skills.
- Highly organized and detail oriented, able to multitask, manage multiple cases independently, adapt quickly to change, and perform well under pressure.
- Proficient in computer and technology skills needed to support documentation, reporting, and daily workflow.
