Position Summary

KLCCOA is seeking a skilled and compassionate Case Manager to provide client-centered services to older adults and veterans. This position is responsible for eligibility determination, needs assessment, service coordination, and compliance-driven documentation in alignment with the Older Americans Act (OAA) and Oregon DHS/APD requirements.

Program Allocation:

50% Veteran-Directed Care (VDC)
25% Oregon Project Independence – Medical (OPI-M)
25% Additional programs

What You’ll Do

Conduct intake and eligibility screenings
Perform comprehensive needs assessments
Develop and monitor service plans
Coordinate services with community partners
Maintain accurate, timely, and compliant documentation
Manage assigned caseload and ensure continuity of care

Preferred Qualifications

Bachelor’s degree in Social Work, Human Services, Psychology, or related field
2+ years of case management or care coordination experience
Experience working with older adults, veterans, or individuals with disabilities
Familiarity with Medicaid, OPI-M, or Veteran-Directed Care programs
Ability to manage complex caseloads independently
