Position Information

Compensation: $20 - $23 /hr
The Bridge for Youth utilizes an equitable & competitive compensation program and salary structure that includes regular Cost of Living Adjustments and raises. For this position, the salary range is $20 - $23 per hour, depending on experience. This salary is non-negotiable beyond this range to counter pay inequality and uphold internal parity for salaries.
Location: 100% on-site at Rita’s House location in Minneapolis
Working Hours: Sun-Thu: 10:30 AM – 7:00 PM. Some holidays included. Position participates in on-call back-up rotation, as necessary and assigned. Rotation of on-call duties is 24 hours a day for seven days.

Position Overview

For over 50 years, BFY has been at the forefront of addressing youth homelessness in Minnesota, positively impacting the lives of the most vulnerable youth in our community, through youth-centered and culturally-responsive programs that offer a pathway out of homelessness, including the most marginalized – minors, BIPOC, LGBTQ+, young families, and SEY.

The Case Manager in Rita’s House serves youth aged 18-21 who have been homeless or precariously housed and are transitioning to independent living within a 12-unit supportive housing intentional community. This position works closely with the Transitions Coach, property management (White Cap Property Management), and community partners to assist youth in obtaining education & employment and familiarizing youth with community resources.

Key Responsibilities

  • Responsible for case management of 12 youth including aftercare youth, providing services and resources as needed
  • Responsible for conducting intake and exit for youth
  • Collaborate with Property Manager (White Cap Property Management), HR & Admin Director (Facilities), Finance Director, Grant Manager, and Data & Evaluation Manager as needed
  • Facilitate weekly house meeting with residents of Rita’s House
  • Make appropriate referrals
  • Conduct weekly consultations to assess and address needs of residents
  • Conduct the intake and exit process
  • Partner with property management and Transition Coach to ensure programmatic success
  • Foster youth relationships with BFY supportive service providers (i.e., oral mental health care providers) and external agencies
  • Provide programmatic input into the Homeless Management Information System (HMIS) system
  • Familiarity with the Hennepin County Coordinated Entry system and process
  • Provide aftercare support to youth
  • Manage the environment to ensure home is functioning and presentable
  • Foster a safe environment to ensure that youth are complying of BFY and White Cap Property Management’s policies and expectations
  • Compile monthly programmatic activity overview report for internal review
  • Stay current on BFY annual trainings
  • Provide supportive services with youth upon discharge and provide aftercare support
  • Maintain youth files
  • Document statistical information as needed
  • Ensure that services are delivered with cultural sensitivity, proficiency and competence
  • Obtain and maintain knowledge of record keeping in a digitized environment, expertise in text messaging, and other digital communication methods

Qualifications/ Required Skills

Required

  • Must have at least one of the following: two years of experience working with youth and/or young adults in a similar setting or associate’s or bachelor’s degree in human services related field
  • Demonstrated ability for commitment to diversity
  • Demonstrated ability to work individually and with a team
  • Demonstrates good communication skills orally and in writing
  • Experience and ability to work with Microsoft Office applications (Word, Excel, Access, PowerPoint & Outlook) as well as Client Record Management (CRM)
  • Hands-on knowledge of record keeping in a digitized environment, expertise in text messaging and other digital communication methods
  • Must be 21 years of age or older
  • Must not have been a resident of any BFY program within the last three years
  • Must clear a background check
  • Must have a valid driver’s license, satisfactory driving record, and insurance limits of $100,000 per person/$300,000 per accident

Preferred

  • Lived experience similar to the youth served by BFY
  • Bi-lingual (English-Spanish) with high proficiency of verbal and written skills
  • Bachelor’s or master’s degree in psychology, social work, or related human services field
