CASE MANAGER-RN HOME HEALTH

Overview:
Receives patient referrals from outside sources and prepares information for admission to homecare. Coordinates the care of assigned patients among a multidisciplinary team to assure quality and cost effective care.
Responsibilities:
PRINCIPAL DUTIES AND RESPONSIBILITIES (*Essential Functions)
  • Assesses admission paperwork and plan of care for completeness, and coordinator corrections as needed.
  • Coordinates care of assigned patients and patient care conferences among a multi-disciplinary team.
  • Schedules field staff visits in absence of scheduling coordinator.
  • Demonstrates working knowledge of current Homecare regulations and departmental policies.
  • Obtains payor authorization and continually communicates with payor sources and field staff to ensure authorization for services.
  • Receives patient referrals from outside sources and prepares demographic and admission information for field personnel.
  • Inputs information into EMR (Electronic Medical Record) after determining appropriateness for admission.
  • Performs other duties as needed and/or assigned.
Qualifications:
JOB SPECIFICATIONS (Minimum Requirements)

KNOWLEDGE, SKILLS, AND ABILITIES
  • Demonstrated excellent oral and written communication skills.
  • Computer proficient.
  • Must be detail oriented and organized.
  • Ability to establish priorities, work independently and with a team, and communicate effectively with multiple team members.
  • Ability to provide professional, courteous, a positive attitude, and to be respectful of others.

EDUCATION
All post high school education must have been obtained from a recognized College or University.
  • Associates Nursing Required
  • Bachelors Nursing Preferred

EXPERIENCE
  • 3 years Home Healthcare or Acute Care experience Required

LICENSE/CERTIFICATION
  • Active Licensed Registered Nurse
  • Basic Life Support Certification
