Summary:
This full-time position provide services in the Behavioral Health Unit, specifically providing service facilitation, as outlined in Wisconsin Administrative Code DHS 36. The purpose of this position is to assist consumers with environmental and supportive services designed to address and overcome financial, personal health, or family issues. Service Facilitators conduct individual and/or family assessments to determine service needs; provide agency services when appropriate; and refers consumers for other community-based services as determined.
This position will also provide services in support to the Children’s Long-Term Support (CLTS) Waiver program services and/or Children’s Community Options Program (CCOP). This includes family-centered assessments, service coordination, and case management services to families who have child(ren) with developmental disabilities, physical disabilities, severe emotional disturbance, mental illness, or severe behavioral problems. The position monitors services with an emphasis on evidence-based practices as well as building natural supports with the family. This position provides outreach and aftercare activities as well as other duties as assigned.
This position will provide Access Intake duties on a rotating basis for the Behavioral Health Unit as well as provide after-hours crisis response between the hours of 4:00 pm and midnight.
Essential Functions of the Position:
· Position fulfills the requirements of CCS Service Facilitator as outlined in Wisconsin Administrative Code DHS 36.
· This position will participate in Children’s Long Term Support (CLTS) Waiver training and provide CCS Service Facilitation for dually-enrolled CCS and CLTS consumers.
· For the CLTS program, assist families and children in the initial application and eligibility process and provide service coordination when a child is receiving waiver services. This includes working with the family to develop an Individualized Service Plan, which will include the services and supports needed to address the child and families specific needs.
· Implement wraparound services in support of recovery concepts and practices.
· Accepts requests and referrals for agency services.
· In collaboration with the consumer and their supports, conducts comprehensive, strength-based assessments through interviews, home visits and collecting of collateral information and data.
· In collaboration with the consumer and their supports, determines the range and type of services needed for preventative or rehabilitative services.
· In collaboration with the consumer, their supports, and providers, assesses ongoing consumer needs while developing and updating service plans to address said needs.
· Explains the scope of services and discusses consumer’s rights and responsibilities in relation to the use of services.
· Arranges or provides for appropriate services based on assessed needs.
· Provides crisis and short-term interventions for consumers and families.
· Provides support as necessary for consumers and/or family or identified supports in individual and/or group formats.
· Works efficiently to achieve treatment objectives without duplication of effort and with maximum effective use of time and resources.
· Attends and participates in staff meetings, supervision, and other regularly scheduled meetings and all-agency meetings as scheduled.
· Completes and maintains consumer and program-related documentation via electronic health record in a concise, thorough, and timely manner.
· Maintains confidentiality of records and information related to consumer’s treatment.
Access Intake:
· Participates in the Access rotation.
· Receives and documents reports and forwards to Supervisor or their designee.
· Provides information and resources to those contacting the Department.
· Conducts initial interviews in order to determine the nature of access or service need and what Unit or Department is best able to assist the consumer.
On-Call:
· Responds in-person to mental health emergencies after-hours for the purposes of assessment, intervention, and response planning.
· Makes decisions based on the determination of need for immediate safety; coordinates with consumers, support systems, families, and other professionals regarding response plan decisions as well as and coordinates with emergency placement settings.
Qualification Requirements:

Education and/or Experience:
· Bachelor’s Degree in Social Work, Psychology, Human Services, Criminal Justice or closely related field.
· Current Social Worker Certification preferred, but not required.
· Previous background or employment history aligned with human services, case management, or intake and referral service industry.
· Ability to pass required background checks.
· A valid driver's license.
· A minimum of 6 months working with people, children and/or adults, challenged by chronic mental illness and/or substance use is preferred.
· Familiarity with DHS 36 Wisconsin State Statutes is preferred.
· Knowledge of Nightingale Notes or other electronic health record systems is preferred.
Training will be required to meet standards for specific job duties.
