As a Medical Social Worker at Ehsas Home Health, you will play a vital role in providing comprehensive support to patients and their families. Located in Lincolnwood, IL, this position is integral to our commitment to delivering personalized care that enhances the quality of life for our clients.

Responsibilities

• Conduct assessments to determine patients' social, emotional, and environmental needs.
• Develop and implement individualized care plans in collaboration with healthcare teams.
• Provide counseling and support to patients and families facing challenges related to health conditions.
• Facilitate access to community resources and services that enhance patient well-being.
• Educate patients and families about coping strategies and available support systems.
• Advocate for patients' rights and needs within the healthcare system.
• Document patient interactions and progress accurately in compliance with regulations.
• Participate in interdisciplinary team meetings to ensure holistic patient care.

Qualifications

• Master's degree in Social Work (MSW) from an accredited institution.
• Valid state licensure as a clinical social worker (LCSW) or equivalent certification.
• Experience working with geriatric populations and understanding their unique needs.
• Strong skills in child family counseling and behavioral therapy practices.
• Ability to develop effective treatment plans based on individual assessments.
• Excellent communication skills for interacting with patients, families, and healthcare professionals.
• Knowledge of community resources available for home health patients is preferred.
• Prior experience in home community care settings is a plus.
