Case Manager, Social Worker - De Pere

Job Specifics

  • Location: 1325 Angels Path, De Pere, WI 54115/Hybrid
  • Hours: 40 hours/week
  • FTE Status: 1.0 FTE, Full Time
  • Work Schedule: Will be working within the day shift between Monday – Friday. Monday, Wednesday, Thursday will be working on-site, and Tuesday and Friday will be working remote; with some flexibility.

Job Description:

  • Responsible for addressing the psychosocial needs of patients participating in a Dialectal Behavioral Therapy outpatient program, as well as outpatient behavioral health.
  • Applicant will be responsible for utilizing consultative services, psychosocial assessment, and comprehensive intervention plans. Coordinates the treatment planning process for patients in the pre-, intra-, and post- care settings, as feasible; coordinates follow up services post discharge; collaborates with other members of the treatment team to assure the continuum of care.

Qualifications:

  • Wisconsin Social Work license required.
  • Bachelor's degree in Social Work required; Master's degree preferred.
  • Skills in assessing, planning, and managing patient care in a health care facility recommended; background in care management is beneficial; knowledge of available community resources to assure the continuum of care is beneficial.
  • Training in Dialectical Behavioral Therapy (DBT) treatment, strategies, or principles is ideal but not required. Willingness to be trained in DBT is necessary.
