Case Manager / Support Coordinator

Seeking organized, efficient, friendly individuals to fill professional, full-time, support coordination position serving caseloads of 43+/- individuals diagnosed with developmental disabilities.

Caseload located in Hernando, Marion, and Sumter Counties. Base office located in Wildwood, FL (Sumter Co.).

Support Coordination is the service of advocating, identifying, developing, coordinating and accessing supports and services on behalf of individuals or assisting individuals and their families to access supports and services on their own through the Medicaid iBudget Waiver.

Duties include, but are not limited to:

- Ongoing, monthly communication with individuals on caseload and their supports;

- Monthly visitation with individuals served;

- Maintaining central file for individual served;

- Coordinating annual support planning meeting for individuals served;

- Communication with State and outside agencies to provide supports and services;

- Identifying appropriate community supports and services for individuals' needs.

Minimum requirements:

- Must have at least a bachelor's degree in developmental disabilities, special education, mental health, counseling, guidance, social work or health and rehabilitative services;

- Must have at least two years professional experience in developmental disabilities, special education, mental health, counseling, guidance, social work or health and rehabilitative services.
