service-relatedSupport Coordinator Responsibilities
A Support Coordinator is qualified by the Division of Developmental Disabilities (DDD) to provide services that assist participants in gaining access to needed program and state plan services, as well as needed medical, social, educational, and other services. Support Coordinators complete a variety of responsibilities in partnership with the person, his/her family, DDD, and service providers, which include:
Identifying the person's support needs and preferences
Developing the NJ Person-Centered Planning Tool and NJ Individual Service Plan
Locating options for services that include: traditional disability providers, generic community supports, government supports beyond DDD, and/or natural support based on funds available in the person's individual budget.
Ongoing monitoring of supports and services by conducting monthly calls and quarterly face-to-face visits.
Responding to emergencies and other service related needs of the person and/or family
Required Skills
Qualifications
This is a management-level position and not suitable for an entry-level candidate. To be considered, the candidate must have clear management experience.
Bachelor's Degree -- and - 1 year of experience working with individuals with developmental disabilities (experience can be caring for a family member or paid or non-paid work).
The experience must be the equivalent of a year of full-time documented experience working with individuals with intellectual/developmental disabilities;
This experience can include paid employment, volunteer experience, and/or being a family caregiver of an individual with a developmental disability;
If you have previously provided care coordination to a different population and some percentage of the individuals you served had developmental disabilities, you may be able to demonstrate the equivalence of a year of experience working with individuals with developmental disabilities
25% of time will be required for travel up to an hour one way throughout New Jersey for client visits. Must have a reliable vehicle.

Pay: $55,000.00 - $58,000.00 per year
