Case Managers (Family Services Specialist -- Community Schools)

Primary Function: Community Schools Attendance Case Manager assesses the service needs of Case Management Community School Program participants and their children to support development of client competencies, independence, resource connections, and stability in the community setting. The goal of the case manager role is to minimize disruption in school attendance for the enrolled students.

Where:

Gompers Elementary School (19151)

Education: Minimum degree requirement is BSW or BA in Sociology, Psychology, Theology, Education, Criminal Justice, Human services, or Public Administration

Experience: 2 years' experience in human services with at least 1 year in social work or a related area of work that includes case management, education, or child welfare experience working with children and youth, juvenile delinquency, and/or families. Experience in counseling, drug and alcohol and/or mental health services preferred. Demonstrated ability to make assessments utilizing sound judgment is critical. Must have and maintain a comfort level working in community settings, in-home settings and with diverse cultures.

Other: Valid Driver’s license is required
