  • Provide case management services for children ages 11 – 21 with mental health and/or co-occurring disorders as part of the Children’s Community Action Treatment (CAT) Team in Charlotte County.
  • Provide services in the community; including school, home, etc.
  • Interact with community partners and other stakeholders, as appropriate.
  • Hours include evenings and on-call, as necessary or assigned.
  • Conduct assessments, create treatment plans, coordinate and provide treatment and supportive services to children and their families.
  • Provide appropriate referrals for additional services and resources, as identified.
  • Provide support and advocate for children and family’s rights and needs.
  • Assist with development of natural support system(s).

Must meet eligibility requirements

  • Bachelor’s degree in human services field.
  • Minimum one (1) year experience working with children and families.
  • Knowledge and understanding of the characteristics of children with mental health concerns, families, and family systems preferred.
  • Ability to perform repetitive tasks.
  • Ability to sit or stand for extended periods.
  • Ability to safely operate a motor vehicle in all driving conditions.
  • Ability to lift up to 10 pounds.
  • Ability to pass a level II Background clearance and drug test.
  • Valid FL drivers’ license, insurance, and safe driving record.
  • Dependable transportation (registered, safe operating conditions, etc.).
  • Must maintain high standards of ethical and professional conduct, while adhering to agency policies and procedures.
  • Ability to use a computer.
  • Attention to detail.
  • Ability to work independently and as part of a team, in collaboration with other community partners.
  • Ability to manage stressful situations and display appropriate work demeanor and boundaries.
  • Ability to demonstrate excellent customer service.
  • Strong oral and written communication skills.
