- Provide case management services for children ages 11 – 21 with mental health and/or co-occurring disorders as part of the Children’s Community Action Treatment (CAT) Team in Charlotte County.

Provide services in the community; including school, home, etc.
- Interact with community partners and other stakeholders, as appropriate.
- Hours include evenings and on-call, as necessary or assigned.

Conduct assessments, create treatment plans, coordinate and provide treatment and supportive services to children and their families.
- Provide appropriate referrals for additional services and resources, as identified.
- Provide support and advocate for children and family’s rights and needs.

Assist with development of natural support system(s).

Bachelor’s degree in human services field.
- Minimum one (1) year experience working with children and families.

Knowledge and understanding of the characteristics of children with mental health concerns, families, and family systems preferred.

Ability to perform repetitive tasks.

- Ability to safely operate a motor vehicle in all driving conditions.

Ability to pass a level II Background clearance and drug test.
- Valid FL drivers’ license, insurance, and safe driving record.
