CCBHC Targeted Case Management Specialist

We are currently looking to fill a Targeted Case Management Specialist role at our Merrionette Park Office.

SALARY:

  • The average starting salary for this position will fall in the range of $45,000 and $55,000 annually. Where candidates fall in this range will be based on skill and experience level.
    • Final salary will be determined based on years of experience, licensure level, and relevant credentials

$2,500 Sign-on Bonus for first-time employees with MFS! ($1,250 on the first paycheck and $1,250 after 6 months of employment).

ESSENTIAL JOB FUNCTIONS:

  • Provides community targeted case management to CCBHC clients who meet at least one of the following criteria
    • Individuals at high risk of suicide or overdose.
    • Those experiencing an acute crisis episode (needing ongoing crisis stabilization supports).
    • Individuals in critical transitions, such as homelessness or transitioning back to the community from an incarcerated setting (jail or prison).
    • Individuals transitioning from an institutional setting, such as a hospital, nursing facility, or residential treatment center.
    • Clients already enrolled in intensive programs like Assertive Community Treatment (ACT), Community Support Team (CST), or Violence Prevention – CST.
    • Individuals receiving supported housing or supported employment services.
  • Gathers information for the IM+CANS identifying medical, social, legal, educational, and housing needs.
  • Develops specific goals integrated into the IATP (Individualized Assessment and Treatment Plan) related to identified targeted case management needs.
    • Actively leads care planning process that specify direct care resources to meet physical and psychosocial needs; by prioritizing problems and establishing mutually agreed upon goals specific to the client.
  • Documents the care and case management plans and other vital information in electronic health record after each contact and service.
  • Monitors daily alerts for participants entering an emergency room or inpatient hospital processes and other coordinated care interfaces, following up with clients and facilitating post-discharge appointments.
  • Participates as assigned in participant education, including development of materials, conducting presentations or supporting other team members in such efforts.
  • Works with multidisciplinary professionals and community agencies to provide a continuum of coordinated care to meet the complex needs of clients.
  • Participates in agency and site based required meetings and trainings.
  • Other duties as assigned.

KNOWLEDGE, SKILLS, and ABILITIES:

  • Requires mobility to work with a community-based caseload.
  • Ability to maintain assigned community and home-based services.
  • Ability to work effectively with diverse individuals and groups.
  • Experience working with diverse client populations with significant behavioral health needs; social service experience is preferred.
  • Punctual, reliable and dependable with the ability to work flexible hours (some evenings and weekends).
  • Ability to produce high-quality work independently.
  • Demonstrated commitment to social justice, and advocacy

QUALIFICATIONS:

  • Bachelor’s degree in a human services field or 2+ years' experience in the mental health field, social service or healthcare setting, required.
  • Must obtain and maintain IM+CANS certification, required.
  • Bilingual in Spanish, preferred.

ADDITIONAL REQUIREMENTS:

  • Satisfactorily pass all relevant screening and criminal background checks.

TRANSPORTATION/TRAVEL REQUIREMENTS:

  • Driving for work required with transportation of clients preferred in a personally owned vehicle.

PHYSICAL DEMANDS

  • While performing the essential duties of this job, the employee is required to sit and stand for sustained periods of time, frequently talk, hear, use hands and fingers to feel, handle, and operate objects or controls, and occasionally stand, bend, stretch, stoop, kneel and crouch. The employee is required to have close visual acuity to perform an activity such as preparing and analyzing data and figures, transcribing, and viewing a computer terminal.
