EXAMPLE OF DUTIES
Position centrally coordinates and monitors all Special Needs Parole referrals and applications and ensure all statutory criteria are met; ensures adherence to Colorado State Statutes, Departmental policy and processes.Primary job duties and responsibilities include, but are not limited to: Review and prioritize offenders to ensure those with high medical, psychological, and intellectual needs are moved forward in the process if their situation deteriorates;Prepare Special Needs Parole documentation by reviewing all relevant information, and tracking documentation data; retrieves, enters, updates and finalizes offender information within database and ensure the databases are accurate and appropriately secured;Pull data as needed on offenders, reviews offenders to ensure they meet statutory requirements, and completes appropriate documentation for clinical services;Coordinate with facility and pre-release case managers to provide the Parole Board with a complete Special Needs Parole application; assist in developing departmental policy and process regarding Special Needs Parole; Coordinate with other units to ensure offenders have the appropriate benefit coverage upon release;Assist offenders with the individualized transitional pathway as appropriate per their identified risk/needs and in accordance with Colorado Revised Statute and CDOC policy;Provide for timely submission of pertinent documents for community corrections referrals, interstate transfers, parole board hearings, and parole plans in accordance with Colorado Revised Statute and CDOC policies; Work collaboratively with and effectively communicate with the Pre-Release, Community Re-Entry Specialists, Facility Parole Officers, and Community Parole Officers as necessary to identify and provide stabilization resources for the transitioning offender;Assist the offender with preparing for transition, through Inmate Benefit Application and Assistance Program (IBAAP) applications where applicable, reinstatement of veteran’s benefits where applicable, work with the offender to identify potential housing, and support systems in the community, etc;Research case files and prepare plans for presentation to the community and parole boards;Other duties as assigned.
